This form is to be completed by the business and or property owner whose facility has been inspected by a Certified Access Specialist (CASp) on or after January 1, 2017 and was provided a report completed in accordance with Construction Related Accessibility Standards Compliance Act and a Disability Access Inspection certificate (DAIC). Reporting this information to the Division of the State Architect and inclusion on the list of CASp inspected businesses is voluntary.

Business/Property name, facility’s city and zip code and date of inspection will be listed on the “Businesses Inspected by a CASp” found on the Division of the State Architect’s CASp webpage. You can expect to see your business name listed within 1 to 2 business days after your submission.
