VOLUNTARY LISTING OF CASp INSPECTED BUSINESSES

This form is to be completed by the business and or property owner whose facility has been inspected by a Certified Access Specialist (CASp) on or after January 1, 2017 and was provided a report completed in accordance with Construction Related Accessibility Standards Compliance Act and a Disability Access Inspection certificate (DAIC). Reporting this information to the Division of the State Architect and inclusion on the list of CASp inspected businesses is voluntary.

Specify the city that is included as a part of the postal address. For example, Mountain View.
Specify the postal code that is included as a part of the postal address. For example, 94043.
DAIC INFORMATION

Business/Property name, facility’s city and zip code and date of inspection will be listed on the “Businesses Inspected by a CASp” found on the Division of the State Architect’s CASp webpage. You can expect to see your business name listed within 1 to 2 business days after your submission.
